Can the Laser Smart Blue stimulate saliva or regenerate the parotid gland in immunocompromised patients?
Xerostomia results either from radiation-induced reduction in the major glands (e.g. parotid) or from drug-induced reduction in the minor glands. Studies show laser is effective for the radiation-induced type but not the drug-induced type. The laser has a dedicated setting and tips for this purpose.
What is the treatment frequency for photobiomodulation (PBM) for radiation-induced xerostomia?
Roughly three times per week during the radiation period, though visit frequency may be impractical.
What is the penetration depth?
About 2-3 mm.
Can it be used on newborns?
Yes, but not with local or topical anesthetic; use a device to secure the tongue.
What goggles should be used for children?
Pediatric-specific laser safety goggles should be used.
Can the laser be used in rooms that aren't closed off?
The laser wavelengths are non-ionizing and cannot penetrate walls, but everyone within 10 feet must wear the supplied safety goggles. In open-bay setups (e.g. orthodontics, pediatrics) with chairs close together, goggles must be worn within 10 feet, so an isolated chair or operatory dedicated to laser use is suggested.
What are the qualities of the 450 nm wavelength?
For new users, the 450 nm wavelength is more technique-sensitive than other diode wavelengths because it has 2-3x more affinity for hemoglobin, acting mainly at the surface with minimal depth penetration. It is therefore not ideal as a primary wavelength for PBM.
What whitening gel should I use with the Laser Smart Blue?
Two compatible gels: Laser White 20 (Biolase) and PreciseWhite Gel (AMD). Both contain hydrogen peroxide and include a dam material to protect the gums. Laser White 20 may be more effective because it contains a dye that the laser targets, enhancing performance.
Can plastic sheaths or barriers be used over PBM treatment tips?
It depends on the tip and heat generated. Deep-tissue or pain-therapy tips generate heat, so barriers are generally not recommended unless thermal tolerance has been carefully evaluated. Low-heat wound-healing tips may allow a clear plastic barrier, but verify compatibility first. Where sterilization turnaround is long, follow strict infection-control protocols and don’t reuse tips without proper sterilization or an approved barrier system.
What is the recommended treatment frequency for PBM in pain protocols?
Commonly 2-3 times per week for 4-6 weeks for optimal outcomes. Where scheduling is limited, once-weekly treatment can still provide meaningful benefit versus no treatment, though effects may be slower or less robust.
